Support once a Care home is chosen

At the point where you have decided on your preferred care home choice, your Care Home Support advisor will be there to assist you further.

We can continue helping with the following:

  • Assistance with completing the financial form (including photocopying of documents etc...).
  • Assisting with obtaining Power of Attorney or Court of Protection documents (We would advise you seek additional professional guidance from a legal professional in all cases).
  • Helping you to arrange furniture removal (either to the care home or if you need help to clear the current abode) – Please note that this may need the services of an external contractor for which there will be a charge (set by the contractor, not ourselves).
  • If you have pets that you may need to find a new home for, we can assist with relocating them or will liaise with your social worker to do so.
  • We can help you to prepare your personal Care Plan* (this is usually completed by your social worker, however many 'self-funders' find themselves without this initial support).

It offers the care home/staff a more detailed and personal insight into every aspect of your social and care needs.

Ranging from:

  • What you would like care/nursing staff to assist you with (hygiene needs/baths/showers, grooming etc...).
  • Your preferred diet.
  • Likes/dislikes.
  • Hobbies and interests.
  • Religious/cultural needs.
  • Interests outside of the care home.
  • Anything else that you feel is important to YOU.
